We’re glad you chose Sip Signature Dentistry! As a new patient, we want to make sure you know what to expect during your first visit.
What to bring
When we schedule your initial appointment, we will send you a welcome packet containing new patient forms or, if you prefer, you may download these documents from our website
These forms provide us with important information about your medical history and insurance. Please bring this completed paperwork to your first appointment.
You may already have recent x-rays (within the past 1 year) from your previous dentist. If so, please bring these with you.
Plan on having x-rays if you are not bringing recent x-rays with you. These x-rays will allow Dr. Sip to assess your jaw and tooth structure and any potential problem areas such as tooth decay.
Dr. Sip and a dental assistant will perform a thorough examination to evaluate the health of your teeth, gums, and bite. This initial examination is an important diagnostic and preventative step. During this time, Dr. Sip will learn as much as possible about your individual dental needs. We encourage you to ask any questions you may have about your dental health so that Dr. Sip can address your concerns in your customized treatment plan.
Next, if you require treatment beyond routine cleanings, we provide a complimentary consultation to discuss your treatment plan, fees, scheduling, and insurance and financial arrangements.
Your appointment is important to us and we want to allow adequate time to address all of your needs. Please arrive on time for your initial appointment and allow 1 hour to complete your consultation. We respect our patients’ time and do not overbook appointments. As a courtesy, our office staff will provide a reminder telephone call 48 hours before your scheduled visit.
Dental Insurance benefits and payment
We strive to deliver the highest quality dental care at a cost that is reasonable for our patients. For your convenience, our practice accepts payment via cash, check, credit card (Visa, MasterCard, Discover and American Express) and we are able to work with most dental insurance plans.
